In my profession we often have to take blood glucose levels on certain animals many times. We don’t use anything but their chart to record the levels. It would save time, and be more efficient to use a template like this

This is another thing we don’t provide for our clients. We make 90% of our appointments by phone, so we rarely ever have to give something to a client to confirm an appointment. But we do from time to time, and it would be great to have some of these on hand.

Many times a day we have to send patients home with more than 1 medication, and we always are getting calls asking us questions that this sheet will answer.
