icky poo... You might not want to read...
So I had my breast and pelvic PI exam yesterday. It was interesting. I didn't have a big problem at all with the breast and abdominal part of the exam and even checking for hernias weren't a huge deal. However, the pelvic part was weird. My PI was still at the last stages of her period apparently so it was strange since we had to wipe the blood off her cervix. I kinda felt like there wasn't any way for me to get it clean since as I wiped off the face of the cervix more blood would kinda leak out and be pulled out by the cotton swab since as all you girlies know I'm sure, the blood that comes out isn't like regular blood but has a bit of a mucus-y consistency and so pulling on mucous leads to more mucus coming out. I managed to get it mostly clean though which is about as good as you can expect. Lucky for me when I inserted the speculum the cervix fell immediately into perfect view. It was kinda cool cuz I didn't really expect the cervix to look like that. It's way bigger than I think it is. My poor partner had a few more issues getting the cervix into view. It sorta contorted for him and was at a slanted angle so he had to work for a while to try to get it into a good enough view that if he had to do a real PAP smear it would work.

Some blood definitely came out with the speculum though as I pulled it out. And since I had wet the speculum and a couple fingers with water before doing all the speculum stuff, the blood was extra watery and drippy and I got some on my pants. I definitely have to wash those before wearing them again! There's just something about another woman's vaginal blood and secretions which is freaky to me. My own, I don't think I would care as much (although I am still sorta grossed out by my own menses). Maybe it's just something to get over with more experience. I mean there wasn't a ton of blood on my pants and you can't really see it since it was watered down but I know I dripped on my pants and it just makes me thankful I wore pants yesterday! Again, my partner had a bit less luck and a good bit of blood came out.

Doing the abdominalvaginal bimanual was sorta odd. The PI is trained to show us how to feel for everything and let us sit there and feel around until we feel what we're suppose to feel. I have to say that it was sorta weird to have 2 fingers in a woman's vagina and stand there waiting to get some extra instruction from her on how to feel her ovaries and uterus. I was definitely surprised at how easily I could find the uterus. Again, it was definitely larger than I thought it was (and it probably helped that my PI was a small-ish woman.. well my size). Plus, the placement of the ovaries was different from how I thought it was. My PI was awesome cuz she could tell when we were touching her ovaries and knew her body well enough to tell us which way we had to move and what we had to do to really get to the structures we needed to feel.

The weirder part was doing the abdominal-vagina-rectal exam. As most of you probably know, I'm sorta rectal and fecally phobic (I wonder what the official terms for those would be.. and I guess it's more fecally phobic since I think my fear of rectum more stems from the fear of fecal matter being present but who knows for sure). Again, it was just strange for me to have one finger in her rectum and the other in her vagina listening to instructions and not really doing anything. From that position we had to again try to feel for ovaries and uterus. I couldn't feel the ovaries from this angle although my partner got it better (or at least more quickly) from this angle than he did with just the abdominal-vaginal bimanual. I definitely did get the uterus from here though. I don't know. I've always imagined that getting a rectal done would be a very uncomfortable experience and here she was sitting there patiently telling us to wiggle around and move our rectal finger around to try to feel whatever and didn't seem pained at all. I guess I know that people have anal sex and stuff but in my mind I still see it as a potentially uncomfortable thing that maybe in a sexual context could feel good but I dunno. Anyways, yeah, lots of rectal finger wiggling. Ick. I was talking to a classmate during lunch before the exam who said that he didn't think any fecal matter would be present in the rectum. Yeah, he was definitely wrong. My finger def came out with poo on it. After I degloved I asked the PI if I should wash my hands again before doing sorta the closing stuff with the patient. She said that it wasn't necessary since I would be going to wash my hands in the next patient's room in a bit and that since I had gloves on it didn't really matter. I dunno. I definitely felt unclean and definitely washed my hands a couple times before I did much else and even then I still felt unclean. I def don't think things requiring rectals are for me... This was where my partner did better. He was actually able to find the ovaries from this position. I couldn't do it but I figured that as long as I felt it vaginally it wasn't a huge deal if I couldn't find it rectally. Plus, I didn't wanna keep my finger in there too much longer than necessary.

At least that's over and I won't have to stick my finger in anyone's vagina or butt for a long time (hopefully)!
